The Western Health and Social Care Trust (Western Trust) is to promote the new Acute Care at Home Service on International Day of the Older Person taking place on Monday next, 1 October.
Under the new service, patients can be referred to the service by their GP who will contact the Acute Care at Home team who will facilitate a discussion between the GP and the Consultant who will agree the best place of care to meet their specific needs.
Patients may be offered the service, involving acute care for an anticipated period of around seven days, from a specialist multi-disciplinary team consisting of consultant geriatrician, speciality doctors, occupational therapist, physiotherapist, nurses and a pharmacist.
Alan Corry Finn, Western Trust Director of Primary Care and Older People Services said the initiative was “further reform” of primary care and older people services focussing on Care in the Community.
He added: “This new service will develop and expand the range of services and treatments provided within primary care setting.”
